The preoperative condition was not simply a matter of excess fat.
The lower eyelid showed a combination of bulging and volume imbalance,
which required repositioning rather than removal.
Aggressive fat removal in this type of case
often leads to hollowing and long-term instability.
Why We Did Not Remove More Tissue
One of the most important decisions in this case
was not how much to remove,
but how much to preserve.
Excessive removal of fat or skin
can create tension and increase the risk of retraction.
Instead, the approach focused on maintaining volume
while redistributing it into a more stable position.
Structural Support Was Essential
Fat repositioning alone is not sufficient
if the supporting structure is weak.
SMAS fixation was performed
to reinforce midface support
and reduce downward tension on the lower eyelid.
This plays a critical role
in preventing late changes such as retraction or recurrence.
Results That Improve, Not Deteriorate
At postoperative day 7,
swelling and temporary asymmetry were present.
At three months,
the contour became more refined.
At one year,
the result appears stable,
natural,
and balanced with the surrounding structures.
This progression reflects a recovery process
that leads to improvement over time,
rather than deterioration.
