This patient traveled to Korea seeking revision eyelid surgery after becoming dissatisfied with the results of a previous procedure.
The original surgery had created a crease that was positioned too high and fixed too deeply. As a result, the eyes appeared overly defined, giving the face a heavier, more feminine appearance that did not match the patient’s desired image.
His goal was not to have larger eyes. He wanted a more natural, masculine appearance similar to the subtle eyelid crease often seen in Korean male celebrities.
The Challenge Was Not Simply Lowering the Crease
Many people assume that high fold revision simply means creating a lower eyelid crease.
In reality, revision surgery is far more complex.
When a crease has been deeply fixed by a previous operation, the first priority is releasing the existing scar tissue and adhesions that continue to pull the eyelid into an unnatural position.
Without adequately releasing the previous fixation, lowering the crease alone often leads to multiple folds, persistent asymmetry, or an unstable result.
For this patient, restoring the normal eyelid structure was the most important step before creating a new crease.
A Masculine Eyelid Requires Structural Balance
Many male patients do not prefer a deep, highly visible double eyelid crease.
Instead, they often seek an eyelid that appears natural, balanced, and harmonious with the rest of the face.
For this reason, every revision must be planned according to the patient’s anatomy rather than a predetermined crease height.
In this case, the crease was lowered as much as the existing tissue safely allowed while preserving eyelid function and long-term stability.
Four Years Later
Four years after surgery, the eyelid crease remains stable without recurrence of the previously high fold.
The eyelids appear softer, more natural, and better balanced with the patient’s facial features.
More importantly, the improvement has remained stable over time rather than changing during the years following surgery.
For us, this long-term stability is one of the most meaningful measures of a successful revision.
