When too much skin is removed in an effort to tighten the eyelid completely, excessive tension may be placed on the lower eyelid margin.

How Excessive Skin Removal Creates Downward Tension

The lower eyelid is supported by a complex balance of skin, muscle, connective tissue, and lateral canthal structures.

When excessive skin is removed, the remaining tissue must compensate for the lost length.
This creates downward tension that gradually pulls the eyelid margin away from the eye.

As this tension increases, patients may develop:

  • Lower eyelid retraction

  • Scleral show

  • Ectropion

  • Chronic irritation or tearing

These complications are not always immediately visible after surgery.
They often appear gradually as scar tissue contracts and the eyelid adapts to the shortened skin.

Why Skin Deficiency Makes Revision Surgery So Difficult

Among all complications in lower eyelid surgery, skin shortage is one of the most difficult problems to correct.

Unlike excess skin, which can be removed, missing skin cannot easily be replaced.
Once significant skin deficiency occurs, revision surgery becomes far more complex and may require advanced reconstructive techniques.

For this reason, experienced surgeons approach lower eyelid surgery with restraint.
The goal is not to eliminate every wrinkle, but to preserve sufficient skin to maintain natural eyelid function and long-term stability.

In revision surgery, preventing skin deficiency is often far easier than trying to correct it later.
