Why Lowering the Crease Alone Often Fails in Revision Surgery.

Many patients seeking revision surgery believe that lowering the double eyelid crease will solve the problem of a thick or heavy fold.

However, crease height alone is rarely the true cause.

In revision cases, a thickened fold is often the result of unstable eyelid mechanics.
When eye-opening function is insufficient, compensatory forces continue to act on the upper eyelid during blinking and eye opening.

If the crease is lowered without addressing this functional imbalance, the same forces remain.
As a result, the fold often thickens again over time, even after technically successful revision.

This is why some patients experience repeated dissatisfaction despite multiple crease adjustments.

In revision surgery, appearance cannot be separated from function.
Without stabilizing eyelid movement, design changes alone are unlikely to produce lasting improvement.

Lowering the crease is sometimes necessary—but it should follow functional correction, not replace it.
