As men enter their 40s, many begin to experience discomfort caused by drooping eyelids and changes in eye appearance.
When the upper eyelids sag, the eyes can appear smaller and feel heavy, creating a tired or dull impression. When ptosis (eyelid drooping due to weakened lifting muscles) is present, the eyes may look sleepy or fatigued, and in more severe cases, functional issues such as obstruction of the visual field can occur.

Preoperative Condition

In this case, the patient was using his forehead muscles to open his eyes, which caused the eyebrows to lift upward and resulted in a tired, unfocused appearance.
Although the eyes appeared open at first glance, the actual strength of the eyelid-opening muscles was weakened, and ptosis prevented the eyes from opening fully.

In such situations, upper eyelid surgery alone cannot address the root cause.

Surgical Approach

Based on the patient’s individual eye condition, an incision was made to precisely expose the eyelid-opening muscle. The muscle function was then carefully adjusted to allow the eyes to open naturally and comfortably.

Rather than creating an artificially strong fixation, this method allows the eyelid crease to form and remain stable through the eye’s natural movement.

Recovery Process

Swelling and bruising may appear immediately after surgery, which is a normal part of the healing process.
Stitches are typically removed around 7 days after surgery. Once the stitches are removed, the foreign-body sensation subsides and major swelling begins to decrease.

Most patients can return to daily activities within 1–2 weeks.

  • Week 1: Swelling and bruising are most noticeable
  • Weeks 2–3: The eye shape gradually stabilizes and looks more natural
  • 1–3 months: Eye-opening strength and comfort continue to improve
  • Around 6 months: Final eye shape is achieved

During recovery, patients are advised to avoid rubbing the eyes or engaging in strenuous exercise, and to attend regular follow-up visits to monitor healing.

 

 

 

Postoperative Changes

After eyelid contour correction surgery, the eyes open more comfortably without the need to use the forehead muscles.
The overall impression becomes clearer and more defined, yet remains natural. The previously tired appearance improves, resulting in a calmer, more confident, and trustworthy look.

Comparing the amount of visible iris before and after surgery shows a clear difference.
