Why Fat Repositioning Does Not Eliminate Pigment-Based Dark Circles.

Dark Circles Are Not Always a Structural Problem.

Many patients believe that lower eyelid fat repositioning will remove all under-eye darkness.
However, not every dark circle is caused by bulging fat or structural imbalance.

Some shadows are created by volume differences and ligament tension.
Others are caused by skin pigmentation, vascular visibility, or thin skin quality.

These are fundamentally different conditions, and they do not respond to surgery in the same way.

What Fat Repositioning Actually Changes.

Fat repositioning is designed to smooth the transition between the lower eyelid and the midface.
By redistributing existing fat, we reduce bulging contours and soften the shadow created by structural depth.

When performed correctly, the eye area appears more balanced and less hollow.
The goal is not to remove volume, but to restore continuity.

This improvement often makes the under-eye area look brighter—but this is not the same as removing pigment.

Why Pigment-Based Dark Circles Remain.

Pigment-related dark circles originate from skin coloration rather than structure.
Even after successful surgery, underlying pigmentation can remain visible.

Patients may notice that the bulging contour improves while the skin tone itself does not completely change.
This does not represent a surgical failure—it reflects the true nature of the condition.

Understanding this distinction is essential before deciding on surgery.

Structural Improvement vs. Skin Quality.

Lower eyelid surgery corrects anatomy.
It does not replace dermatologic treatment.

Skin tone, vascular color, and pigmentation often require different approaches such as skincare, energy-based treatments, or time.

Expecting surgery alone to resolve pigment-based darkness can lead to unrealistic expectations.

Our Perspective at Ahnsungmin Plastic Surgery.

Fat repositioning can reduce structural shadows, but pigment-related dark circles are a separate issue.

Our goal is to create long-term stability and natural balance—not to promise complete elimination of every visible concern.
