[Difference between fat removal and fat repositioning]
What causes under-eye bulge?
Simply put, when the orbital septum that encases fat tissue weakens due to aging, fat starts to sag, resulting in eyebags. As the fat pushes down, the upper eyelid is also affected, leading to a sunken appearance. Genetic factors play a part, but mostly aging is the main culprit for laxity in membranes encasing the skin, muscle, and orbit.
Many would consider fat removal to correct under-eye bulging. While fat removal addresses excess fat, it is rare to have enough fat that needs to be removed.
Fat removal could be a dangerous approach when patient’s age and the degree of aging is considered.
When there is sufficient under-eye fat, it could be repositioned to fill in the sunken area and enhance volume.
Fat repositioning is not just about removing the bulge; it also provides proper volume and tightening to the area.
Therefore, it is recommended to adjust the surgical process based on the amount of fat, rather than simply removing it.

Before and after fat repositioning for the patient in her 40s
Fat removal should be performed with careful consideration.
When too much fat remains after being repositioned to the sunken area, the remaining fat should be removed to prevent bulging.
However, once the fat is removed, it cannot be restored. Fat removal at an early age may lead to a hollow appearance after the middle age.
Patients end up depending on fat grafts or filler injections to the sunken under-eye area after the excessive fat removal, which can result in a vicious cycle of repeated surgeries.
Therefore, our goal in Ahnsungmin Plastic Surgery is to restore volume in the sunken area through fat repositioning while recovering the under-eye smile muscle.
