Eyelid Hollowing Is Not Always Caused by Aging Alone.

Eyelid hollowing is often attributed to aging and volume loss.
While this can be one contributing factor, it is not the only cause.

In many patients, eyelid hollowing develops due to functional imbalance,
chronic compensatory effort, or structural changes following prior surgery.
When these underlying issues are not addressed,
the problem tends to persist—or recur—regardless of how much volume is added.

For this reason, durable improvement depends on identifying
what is truly causing the hollowed appearance.

In selected cases, orbital fat repositioning can be an effective solution.
By redistributing the patient’s own remaining orbital fat while preserving its blood supply,
support can be restored without relying on unpredictable graft survival.

However, no single technique is appropriate for every patient.

All surgical decisions ultimately belong to the patient.
What matters most is not the choice of procedure itself,
but understanding the cause of the problem
and choosing an approach that addresses it at its root.

Without treating the underlying mechanism,
even well-executed surgery may fail to provide lasting stability.
